A one-page team performance review template
Most review templates collect information. This one forces a decision. It fits on a single page, takes about twenty minutes to fill in with a team, and ends with something you can check on a named date.
How to use it
- Fill it in about one result, not about the team in general.
- Name one constraint. If you cannot choose between two, write both and gather evidence before spending money.
- Every action must address that constraint. Anything else belongs on a different page.
- Always fill in the box marked what we stop. A plan with no removals is a wish list.
- Put the review date in the calendar before the meeting ends.

Let the diagnostic fill it in for you
If you are not sure which constraint to write in box one, run the free diagnostic. It asks short rating questions and returns the top three constraints, the actions for each, what to stop, and the signal to watch — as a plan you can download or print in this same shape.
